吳為吉教授是台灣在早產兒視網膜病變的先驅及開拓者,也是國際上這個領域的知名學者。早產兒視網膜病變是嬰幼兒失明的主因,之前在台灣並無人專注在此領域研究。吳為吉教授從林口長庚醫院眼科部完成5年住院醫師及視網膜研究員的訓練,並且從臨床醫學研究所博士班畢業之後,到美國密西根州William Beaumont Hospital, Royal Oak,從事博士後研究,專精於小兒視網膜疾病的學習。

吳為吉教授自2006年從美國學成回國已為超過700個產兒施行雷射及眼內注射,並施行超過300台玻璃體切除手術。在關於早產兒視網膜病變的自然病史、流行病學、致病機轉、微創手術、抗新生血管物創新治療、視網膜OCT (Optical coherence tomography)、人工智慧、視力復健及長期解剖及功能成果及病人神經發育皆有論文創新發表。

吳教授對早產兒視網膜病變的治療發展出多項創新與改良,獲得領先世界水準的成績,發表國際SCI學術論文220多篇,並擔任American Journal of Ophthalmology Case Reports編輯,獲邀在國際及海外醫學會議演講200餘場,同時進行相關之基礎研究,帶動相關醫療科技之發展,對增進全民健康、提升醫療水平有卓越貢獻。

吳教授於2022年入選全球前2%頂尖科學家、2021年獲第14屆亞太玻璃體視網膜會議最佳論文獎及美國眼科醫學會成就獎、2020年獲國際外科學會金牌獎及銅牌獎,愛爾康優秀論文獎及東京SEED隱形眼鏡論文獎銅牌獎、2018年獲得Azad教授獎(專門對早產兒視網膜病變有特殊貢獻) 、2016年獲得亞太眼科醫學會成就獎、以及香港國際眼科醫學會Travel Award 、其他獲獎還包括愛爾康優秀論文獎金牌獎、國際外科學會佳作獎、世界早兒視網膜病變會議論文第一名等知名獎項肯定。

四歲活潑可愛的程小弟回到眼科門診時,由於他在眼科經常回診,和眼科醫師及護理師很熟悉,他還跑過來給眼科醫師抱抱,除了身材略為瘦小並且帶著眼鏡,其實若不告訴你,你一定無法想像這個小朋友出生時體重不到900克,懷孕週期僅有27週,之前還曾因早產兒視網膜病變,雙眼需接受雷射治療。

早產兒視網膜病變是因早產,造成視網膜血管發育異常不完整,嚴重時會導致視網膜缺氧,血管內皮細胞生長因子 (vascular endothelium growth factor; VEGF)過度表達造成血管新生,而新生之血管拉牽視網膜造成視網膜剝離(retinal detachment),嚴重時會造成失明。

所幸目前在治療早產兒視網膜病變已有長足的近步。除了傳統的雷射治療,現在使用抗血管內皮細胞因子(Anti-VEGF)眼內注射,除了效果與傳統的雷射治療相當,也被發現有減少視網膜破壞,減少日後產生近視,散光,及斜視的優點。在雷射或眼內注射治療後,僅有少數的病人會繼續惡化到需要眼內手術,因而失明的機率已可大幅減少。

但這些過去有早產兒視網膜病變病史的病人和正常足月兒比較,眼睛的結構及功能上其實是有許多細微的差異。這些差別包過較多近視,散光,斜弱視的盛行率及較低視力等。在視網膜及脈絡膜的發育也有細微異常。長期來說,青光眼,白內障,視網膜破洞,視網膜剝離的發生率在這些病人也會較高。所以這些病人最好是能長期追蹤,以便若發現有異常時可以早期治療。

這些「慢飛天使」因先天發育較不完備,所以起始條件較差。這些小朋友需要多費心照料,家長不要給小朋友使用手機及平板,多鼓勵小朋友從事戶外活動及運動,以及注意營養。之後經過適切的治療及復健,這些病人也可以有近乎正常的視力以及神經發育。觀察他們從小到大,有些小朋友進步的幅度空間可真會令人大吃一驚呢!
